Check the previous passport first

PPTC 054 is the adult simplified renewal form for eligible Canadians applying in Canada or the United States. Check the previous passport before filling anything in. Its issue date matters, not just its expiry date.

The previous passport must have been issued within the last 15 years, when you were at least 16, and have had five- or ten-year validity. The name, date of birth, birthplace and gender identifier must meet the official unchanged-details requirements. A child passport cannot be renewed through this route.

If those conditions do not match your situation, use the official eligibility guidance to find the appropriate application. Do not change an answer simply to pass a renewal check.

Confirm that the editor supports your case

Wayform supports a narrower set of cases than the government’s full renewal service. This editor requires an available, undamaged passport and supports the stated Canada or U.S. address scope, two previous addresses and four occupation entries.

Lost, stolen, inaccessible, damaged or reported-found passports, identity changes, special assistance, additional declarations, extra sheets and unsupported spelling are outside this editor. An editor limit does not mean that the government has no renewal option for you. Follow the official route when your case needs more.

Ask two suitable people to be references

Each reference must be at least 18, have known you for at least two years, agree to the use of their details and meet the government’s relationship restrictions. Ask them before entering their information, and confirm that their contact details are current.

Spouses, common-law partners, immediate family and specified extended-family relationships are excluded. Check the full official definition when a relationship is unclear; different surnames or separate addresses do not by themselves establish eligibility.

Choose people who can respond if contacted. References help confirm identity; they are not guarantors. A simplified passport renewal does not require a guarantor.

Review both pages before signing

Compare the passport number and dates with the passport. Account for the requested two-year address and occupation history, and check the references, mandatory emergency contact, validity choice and passport-return choice. Do not omit periods because the available rows are full.

The form permits typed capital letters; handwritten entries use black or dark-blue ink. After printing, sign the applicant areas on both application pages, keeping the first signature within its border. Read the instructions for photos, valid passports and other items to include.

Check the current official submission instructions when assembling the packet. Government online renewal is a separate application process with its own requirements.
