No legal representative, loss/theft/damage, name or identity change, overseas application or government online submission. Answers must be in Vietnamese with every name and diacritic preserved.

Print the complete TK01 on white A4 paper using Fit to printable area so the original scan is not cropped. Preserve its QR and printed text; bring separate correctly sized photos. Read the declaration; personally add the signing place/date, signature and full name by hand. Follow the provincial immigration office’s current arrangements for direct paper applications. Bring the recent 4 × 6 cm photos required for both photo spaces, your latest passport (including a still-valid passport), and your citizen/identity document, electronic identity or valid passport for identification. Pay the current government fee separately. Official photograph/fingerprint capture may be required for a first chip passport. Complete any family date that is only partly known by hand, without guessing. Follow the official pickup or postal-return process. This editor does not create an online government application or book a visit.

Use recent 4 × 6 cm colour photos taken within six months: white background, face forward, uncovered head and ears, no spectacles and appropriate clothing. Follow the two photo spaces and the receiving office’s instructions.
