After you download. Print the complete original page at actual size on A4 paper. Read the declaration, sign inside the specimen box and sign/date the application declaration by hand. Have an identity witness who has known you for at least one year personally complete and sign their entire section and endorse one photo with your full name, signature and date. Bring two recent 35 × 45 mm photos, your expired passport, birth certificate and citizenship certificate, plus a certification letter explaining your travel purpose. Return the expired passport. Lodge personally at the Immigration office, Kumul Highway, Tamarama Building, Port Vila. Normal adult service is listed at VT 10,000 and up to 21 days; confirm current fees and arrangements with the office. Government fees are separate. Loss/damage reports and surcharges do not apply to this expired-only route. Leave office-use areas blank.
Two identical recent photos, less than six months old, 35 × 45 mm. The identity witness writes your full name on the back of one photo and signs and dates it. Attach the photos as directed by the Passport Office; do not cover the specimen-signature box.
