Laser-print both pages on A4 at actual size. Sign inside both boxes and sign/date the declaration on page 2. Bring your original birth certificate, National Identity Card, copies, any previous passport and required supporting evidence. Obtain a photo acknowledgement from an authorized studio and follow the official appointment and fingerprint instructions. Government fees are paid separately.

Sri Lanka requires a digital photo submitted by an authorized studio or department office. Wayform photos and ordinary printed photos do not replace this step.
