For Korean citizens aged 18 or over applying personally inside South Korea for a new ten-year ordinary passport, with an available, undamaged previous passport and unchanged identity details, including any spouse surname. Men in the military-obligation age group must have completed service or be formally exempt. Supports personal pickup. First passports, minors, loss/damage, identity changes, remaining-validity replacement, overseas applications, proxy applications, Braille requests and other military-status cases use the official service.

Read both pages. Print in colour on white A4 paper at 100% actual size (the form specifies 120 g/m²); do not resize, fold or damage the form. Complete the declaration name, date and signature in black pen. Decide and complete the administrative-information consent yourself; if you do not consent or records cannot be verified, bring the required supporting certificates. Attend a domestic passport office with valid government-issued ID, one compliant photo taken within six months, your previous passport (required if still valid), any military-status evidence and the current government fee. Bring family-record certificates if they cannot be checked electronically. Leave the registered domicile blank unless the officer requests it; leave the official-use area blank. Follow the office’s pickup instructions and collect within six months of issue. This prepares the paper application only; it does not book or submit an application. Government fees are separate.

Bring one recent colour 35 × 45 mm photo, head height 32–36 mm, following the national rules. Do not use filters, background replacement or AI alteration. The physical photo is supplied separately.
