After you download. Print all four FA 81 pages at actual size on Letter paper (8.5 × 11 inches), including Attachment A. Also print and complete the correct mandatory adult checklist linked on this page. Follow the current ICA checklist and process-change notice where the older form differs: use a current NID birth certificate for a first application, the original previous passport for renewal, the fee receipt and two photos. Have the application certified by a qualified Commissioner for Oaths or court official; attach their ID copy, address and phone number. The certifier must know you for three years and endorse one photo. Sign both small labels on page 1 and complete your declaration and signing date on page 3 by hand. Lodge personally with ID at ICA, Central Government Office, Waigani. Government fees are separate; confirm current charges and timing with ICA.
Two recent 35 × 45 mm full-face photos with a white background and neutral expression. No tinted glasses, smiling or visible teeth. One photo must be endorsed by the same person certifying the application.
