After you download. Read both PA-1 pages and the current Passport Office requirements. Print both original pages at actual size on Legal paper (8.5 × 14 inches), keeping the reverse-side instructions with the application. Bring your original Palau birth certificate, satisfactory identity document and one colour photo. The current checklist requires the original birth certificate for this route. Glue the photo to page 2. Do not sign or date the application until an authorized oath-administering official tells you to; that official must complete the notarization. Leave every official-use area blank. Personally submit the original notarized packet at Palau Passport Office, Ministry of State, National Capitol, Ngerulmud. Pay the government passport fee separately following the office’s current instructions; the published ordinary fee is US$50. This service prepares the form only.
One colour 2 × 2 inch photo taken within six months, clear full face, plain white background, thin photo paper, normal clothing, no hat or dark glasses. Do not retouch appearance. Glue it in the printed photo box after printing.
