I am a New Zealand citizen aged 16 or over, applying for my first passport. I have never held any New Zealand passport or other New Zealand travel document. *
I am completing and signing my own standard application. I do not need a marriage/civil-union name change, deed-poll declaration, third-party assistance or additional statements. *
I do not wear a head covering or headband in my passport photos, and no additional written statement is needed. For other cases I will use the official form. *
An eligible witness aged 16 or over, unrelated and not living with the applicant, will complete the witness section personally and sign/date the back of one photo. They hold a valid NZ passport and have known the applicant for more than one year. *
I will provide two unaltered photos, all required original documents and any English translations, arrange government payment, and complete the signing and submission steps after printing. *
I have confirmed the applicant’s New Zealand citizenship. Any required grant or descent registration is already complete. *
