An eligible witness aged 16 or over, unrelated and not living with the applicant, will complete the witness section personally and sign/date the back of one photo. They hold a valid NZ passport and have known the applicant for more than one year (or since birth if under 12 months). * Yes No

After you download. Print the application pages on A4 at actual size. The consenting parent must read and sign the consent declaration on application page 11 (packet page 21), and consult other parents or guardians where possible. The witness must personally complete application pages 12–13 in their own handwriting, sign/date the declaration and write the child’s full name, their signature and date on the back of one photo. Include two unaltered photos and required original documents/translations. Complete the final checklist and government payment privately. Submit following the packet instructions. An existing passport is cancelled when the application is received; do not send it unless requested.
Supply two identical recent, unaltered photos using the official photo guide. The witness must certify one photo. We do not offer a photo upsell for this application.
