After you download.

Print the application pages at actual size on A4 paper and read all declarations. Sign and date application page 5 by hand, unless you have selected unable to sign. Include two identical, unaltered recent photos; write your name and the date on the back of one. Do not staple, glue or paper-clip them. Complete the checklist and government payment section privately after printing. Submit to the official passport office for your location. Do not send your old passport: it will be cancelled when your application is received.

Use the photo instructions in the original packet. The photo must be an unaltered true image, 35 × 45 mm. Photos, courier costs and government fees are separate from Wayform preparation.
