For a first Jamaican passport, for an unmarried child under 18, completed by a parent named on the birth record. The applicant must already have evidence of Jamaican citizenship. Previous passports, loss/theft, guardianship disputes, unsupported name changes and additional sheets use the official service.

Print the complete packet on Letter paper at actual size. Complete the declaration in section E and sign/date by hand; the applying parent completes and signs section C and the declaration, and the child signs where required for their age. An eligible certifying official must personally complete section G and certify one of two professional photos. Bring citizenship evidence, identification and any required original supporting documents. First-time adults and children aged 3 or over must appear in person; children must be accompanied by the applying parent. Follow the current PICA instructions if the older embedded notes differ.

Provide two identical recent professionally produced photos. One must be certified by the same official who certifies the application. Photo services are separate.
