For a first Jamaican passport, for an applicant aged 18 or over. The applicant must already have evidence of Jamaican citizenship. Previous passports, loss/theft, guardianship disputes, unsupported name changes and additional sheets use the official service.

After you download.

Print the complete packet on Letter paper at actual size. Complete the declaration in section E and sign/date by hand; sign inside the specimen box as well. An eligible certifying official must personally complete section G and certify one of two professional photos. Bring citizenship evidence, identification and any required original supporting documents. First-time adults and children aged 3 or over must appear in person; children must be accompanied by the applying parent. Follow the current PICA instructions if the older embedded notes differ.

Provide two identical recent professionally produced photos. One must be certified by the same official who certifies the application. Photo services are separate.
