For Italian citizens aged 18 or over registered in AIRE with a current address in the Toronto consular district: Manitoba, Northwest Territories and Ontario except the Ottawa district. Ordinary first passports and renewals only, with no passport-inhibiting court order or unresolved citizenship/name issue.

Print the completed page at actual size and sign by hand as instructed by the consulate. If you have children under 18, also sign and date section B personally. Book an appointment through Prenot@mi and attend in person with the required identification, current address and Canadian-status evidence, the required photo and applicable government fee. Follow Toronto’s current document checklist. Official-use fields stay blank.
