Print all five pages on A4 at actual size. Read the declaration and privacy notice; sign the application only in front of the officer. Complete the privacy acknowledgment, date and signature yourself. Leave all official-use areas blank. Attend the competent office under its confirmed admission arrangements with your valid ID plus a copy, one compliant recent photo, previous passport even if expired, the €42.70 passport payment receipt and €73.50 administrative stamp. Government fees are separate and may change. Follow the official collection notice. This prepares a paper application; it does not book an appointment or replace an Agenda-generated packet.

Bring one recent 35 × 45 mm ICAO photo taken within six months, front-facing with a white background. Keep the photograph separate for the officer.
