For Belgian citizens aged 18 or over registered in Montreal’s consular population register and applying there in person for an ordinary passport. This editor uses the official French form. Mobile-kit missions, minors and applications at another post use the official service.

Print both pages on A4 at actual size. Read the privacy information and complete your signing date and signature by hand. The optional seven-year retention checkbox is left blank for you to decide after reading the form. Bring the application to your Montreal appointment with identity documents, proof of any other nationality, Canadian status and current address evidence, plus the fee and return envelope if needed. Do not send the adult application by post or email.

Montreal takes the photograph, fingerprints and digital signature during the appointment. A separately prepared Wayform photo does not replace that step.
