For citizens by birth born in Antigua and Barbuda, aged 18 or over, living and applying there for a standard first passport or replacement of an expired or full passport in the same legal name. Print all six pages at actual size on paper matching the original Legal size (8.5 × 14 inches). Read pages 4–6. Sign inside the box on page 1 and sign/date section 12 yourself. An eligible, unrelated certifier who has known you for at least two years and sees you must complete section 9 and endorse one photo. Complete the supporting-document ticks in section 6 after assembling originals and copies of your birth certificate, any applicable baptismal, marriage or divorce records, and any previous passport for surrender. First-time applicants must attend an interview at the Passport Office. Check submission arrangements and government fees directly with the office; forms are valid for six months after certification.

Two identical, original, unaltered colour photos, 35 × 45 mm, taken within six months against a plain white background. Follow page 5; the certifier endorses the back of one photo.
