Who this form is for

Canadian citizens aged 16 or older applying in Canada or the United States with an eligible Canadian-passport guarantor. Home, mailing, guarantor and emergency-contact addresses must be in Canada or the U.S. The editor supports up to two identity documents, two previous addresses and four occupation entries.

Cases this editor does not cover
  • Children under 16, other overseas routes and occupation-based guarantor or no-guarantor declarations.
  • Unavailable, lost, stolen or damaged travel documents; gender changes; special declarations; third-party assistance.
  • Extra sheets, additional identity documents or answers and spelling that the editor cannot preserve.

Three details to check

Keep the guarantor’s four fields blank

After you sign all three application pages, your guarantor must personally complete the years-known, signature, date and place-of-signing fields. They also certify one photo and the required identity-document copies.

Official instructions

Use separate citizenship and identity evidence

The identity evidence must be different from your citizenship proof. Together, your valid identity documents must show the requested name, birth date, photo and signature; follow the copy-certification instructions.

Official instructions

Account for the full two-year history

Include every address and occupation period requested, including study, retirement or unemployment. If complete answers need more than this editor’s available rows, use the official form and additional-information sheets.

I am a Canadian citizen aged 16 or over, applying in Canada or the USA for a regular passport. *
My home, mailing, guarantor and emergency-contact addresses are in Canada or the USA. I do not need additional sheets, a special declaration, a gender change or third-party assistance. *
I will sign all three application pages, provide citizenship and identity evidence, obtain commercial-photographer photos, and ask my guarantor to complete their required fields and certify the documents. *
My address and occupation entries account for the full last two years, with no omitted periods or facts. *
Both references are at least 18, are neither relatives nor my guarantor, have known me for at least two years and agree to be contacted. *
My requested name is supported by my citizenship proof and identity documents. I will supply any required original marriage, relationship or legal name-change evidence and disclose every former surname. *
Any previous Canadian travel document is available and undamaged, was not reported lost or stolen, and does not need a PPTC 203 declaration. I will enclose every still-valid document and explain any renewal more than 12 months early. *
My guarantor is a Canadian citizen aged 18 or over, speaks English or French, has known me for at least two years, holds an eligible 5- or 10-year Canadian passport (valid or expired no more than one year), and was at least 16 when they applied for that passport. *
My one or two valid government-issued identity documents, together, show my requested name, date of birth, photo and signature. They are different from my citizenship proof. My guarantor will sign and date copies of both sides where required. *
