Who this form is for

Applicants aged 16 or older renewing an available, undamaged New Zealand passport in the same name, using standard service. The identity referee must meet the official requirements.

Cases this editor does not cover
  • First passports, name changes, lost or stolen passports and damaged passports use the official general application route.
  • Urgent cases and emergency travel documents.
  • Answers or diacritics the editor cannot preserve; never remove an accent or macron simply to make an answer fit.

Three details to check

Match your passport name and measure in centimetres

Use the name on your passport and preserve every character. Enter your height in centimetres; height and eye colour are required. Use the official form if this editor cannot reproduce your spelling.
