Who this is for & what to know
Standard U.S. passport applications made in the United States, including first applications and supported adult, teen and child applications. The editor uses U.S. mailing, residence and emergency-contact addresses. Children still need the applicable parent or guardian evidence and consent.
Cases this editor does not cover
- Urgent travel, special issuance and overseas applications.
- Institutional guardianship, extra sheets, answers that do not fit and spelling the editor cannot preserve.
- DS-64 is not included; loss or theft reporting and supporting evidence must follow the official instructions.
Answers are processed temporarily for previews and PDFs, not saved in our database. A draft stays in this browser tab during checkout. Purchase recovery restores access, not your answers. Access lasts 30 days from order creation.
Before your appointment
Enter parent names as requested. The parent section asks for names at birth. Use the requested first, middle and last names rather than automatically copying a parent’s current married name. Review every parent entry before printing. Official instructions
Handle the Social Security field correctly. Provide your SSN if one has been issued. If you have never been issued one, follow the form’s zero-entry instructions and attach the required separate signed and dated statement. Official instructions
Leave the signature and photo for your appointment. Print the application pages on separate sheets. Do not sign or attach the photo until the acceptance agent instructs you. Bring the required original evidence and photocopies. Official instructions
